“A Hot New Republican Lie”: The Government Spends More On Welfare Than Everything Else

You are probably going to start hearing a hot new lie from Republicans soon: The government spends more money on welfare than on anything else, even the military!

This is apparently the conclusion of a new report from the nonpartisan Congressional Research Service (the same organization that recently said that Obama’s supposed “welfare reform gutting” was totally legal!), though in fact it is a claim made by Senate Republicans who are abusing the nonpartisan research of the CRS. Here’s the story in the Weekly Standard, complete with charts from the Republicans on the Senate Budget Committee. Here’s the story in the Daily Caller, which is more upfront about all the material coming from Senate Republicans and not from the CRS. And here’s a Weekly Standard follow-up with some new charts.

They claim that “welfare spending” is the “largest budget item” for the federal government, with the fed spending $745.84 billion, more than is spent on Social Security, Medicare and “non-war defense.” (Hah.) Plus: “In all, the U.S. government, including federal and state governments, spends in excess of $1 trillion on welfare.”

That is a lot of welfare spending! Those poor people must be rolling in dough, right?

In the context of political discussions, “welfare” traditionally (as in pretty much always) refers specifically to Temporary Assistance for Needy Families, or TANF, the federal program that was created in 1996 to replace the Aid to Families With Dependent Children program — also known as “welfare” — that had existed since the New Deal. This is what people refer to when they say “welfare caseloads” and “welfare rolls,” and when conservatives accuse Obama of gutting “welfare reform” they are referring to TANF. The federal government spends $16.5 billion a year on TANF and, combined, the states spend another $10 billion.

Most of the federal budget is “defense” and war spending and Medicare, which should be common knowledge but that fact is regularly obscured by right-wingers who claim to be deficit hawks but refuse to cut defense spending and are scared of proposing real reductions to our programs for old people. This is how you get poll results where people think most of what the federal government spends money on is “foreign aid” and public broadcasting. So this is obviously just an attempt to rebrand “everything else” as “welfare.”

(On a state level, the majority of money goes, unsurprisingly, to healthcare and education. Less is spent on actual “public assistance” than is spent on prisons.)

The con is pretty easy to see when you read the actual CRS report. Senate Republicans are counting 83 separate (and wildly different) programs as “welfare” in order to make the case that the government is spending more on poor people than old people. The majority of this money is Medicaid and CHIP, which are healthcare spending, which is increasing for the same reason that Medicare spending is increasing, which is that healthcare costs are increasing. (And Medicaid is much less generous than Medicare, because it is a program for poor people, not old people.) But so many other things now also count as welfare, including Pell Grants, public works spending, Head Start, child support enforcement, the Child Tax Credit, Foster Care assistance, housing for old people, and much more. They’re also counting the Earned Income Tax Credit, which is, traditionally, the form of “welfare” that conservative Republicans actually support. Basically, all social spending (though specifically not spending on rich old people or on healthcare for veterans with service-related disabilities, which Republicans requested be excluded from the CRS report) now counts as “welfare.”

So we’ve learned that when you count everything — especially Medicaid and CHIP — as “welfare,” it is easy to make it look like “welfare” is very expensive, because healthcare is very expensive. This dumb lie will live forever, and you will hear until the end of your days that “the government spends more on welfare than it does on defense.”

“Believe Me Because I Said So”: Mitt Romney Goes From Etch A Sketch To Sketchy

“Lead from behind” may be a sound bite the Obama administration regrets, but debating from behind is clearly something President Obama is very good at. He got the first debate’s wakeup call while Mitt Romney let the encounter in Denver mislead him into confusing petulance with strength.

For Obama’s supporters, the fact that the president played offense, had a strategy and seemed happy in his work was reason enough for elation. But the most electorally significant performance was Romney’s. Under pressure this time, the former Massachusetts governor displayed his least attractive sides. He engaged in pointless on-stage litigation of the debate rules. He repeatedly demonstrated his disrespect for both the president and Candy Crowley, the moderator. And Romney was just plain querulous when anyone dared question him about the gaping holes in his tax and budget plans.

Any high school debate coach would tell a student that declaring, “Believe me because I said so,” is not an argument. Yet Romney confused biography with specificity and boasting with answering a straightforward inquiry. “Well, of course, they add up,” Romney insisted of his budget numbers. “I — I was — I was someone who ran businesses for 25 years, and balanced the budget. I ran the Olympics and balanced the budget.” Romney was saying: Trust me because I’m an important guy who has done important stuff. He gave his listeners no basis on which to verify the trust he demanded.

Romney’s stonewalling was so obvious that it opened the way for one of Obama’s most effective lines of the evening: “If somebody came to you, Governor, with a plan that said, here, I want to spend $7 or $8 trillion, and then we’re going to pay for it, but we can’t tell you until maybe after the election how we’re going to do it, you wouldn’t have taken such a sketchy deal. And neither should you, the American people, because the math doesn’t add up.” Obama sought to make that point in the last debate. This time he had a metaphor and a story to go with the arithmetic.

Romney also covertly disclosed that he, like George W. Bush before him, has every intention of cutting taxes on the rich. Like Bush, he used stealthy language to try to achieve a great fiscal coverup.

Here was Romney on Tuesday: “I will not, under any circumstances, reduce the share that’s being paid by the highest-income taxpayers.” Here was Bush in 2000: “After my plan is in place, the wealthiest Americans will pay a higher percentage of taxes [than] they do today.”

This really matters: Romney intends, as Bush did, to push for steep tax cuts for the wealthy. His only pledge is that he’ll keep the share of the total tax take paid by the wealthy unchanged, presumably by reducing other taxes too. And this is supposed to lead to lower deficits? How?

The most instructive contrast between Debate I and Debate II was the extent to which Romney’s ideas crumbled at the slightest contact with challenge. Romney and Paul Ryan are erecting a Potemkin village designed to survive only until the polls close on Nov. 6. They cannot say directly that they really believe in slashing taxes on the rich and backing away from so much of what government does because they know that neither idea will sell. So they offer soothing language to the middle class, photo ops at homeless programs to convey compassion and a steady stream of attacks on Obama, aimed at shifting all the attention his way.

For his part, Obama looks strong when he calmly and methodically confronts the exceptionally large philosophical and practical differences that now divide the parties. He looks weak when he fuzzes up those differences in the hope of avoiding conflict. The fight is often asymmetric because Obama speaks for balance — between tax increases and spending cuts to reduce the deficit, between a thriving market and an active government — while today’s conservatives have no interest in balance.

In the first debate, Obama let Romney back into the race by failing to shake his opponent’s self-presentation. But Romney also put himself into contention by pretending to be a moderate, shelving his plutocratic side and hiding his party’s long-term objectives.

In the second debate, the disguise fell. Romney revealed more of himself than he wanted to and asked voters to endorse a radical tax-cutting program without providing them the details that matter. Sketchy is one word for this. Deceptive is another.

“Aiding And Abetting”: How The Press Helped Mitt Romney Reinvent Himself

Analyzing the presidential campaign in the wake of the first debate, Time’s Mark Halperin wrote on October 10 that Mitt Romney’s sudden “rush to the center” politically had emerged as the key topic – “the central tactical issue”– for the Barack Obama’s team to address. Halperin stressed it would be a challenge for Democrats because the Romney’s campaign’s “brazen chutzpah knows no bounds.”

How odd. At the first debate Romney had so brashly reinvented himself by shifting his position on taxation, immigration and health care away from the Republican Party, that the onus was on Obama to counter Romney’s slick maneuver. In other words, Romney’s flip-flops, according to Halperin, were a major problem for the Obama campaign, not for the Republican who late in the game unveiled a new political persona. (Farewell “severely conservative.”)

It’s also telling that on October 10, Halperin considered Romney’s makeover into a moderate to be the campaign’s dominant issue. Yet one week earlier on the night of the first debate when Halperin graded both participants, the pundit made no reference to Romney’s “rush to the center.” In real time, Halperin heaped praise on Romney’s style “(Started strong, level, and unrattled — and strengthened as he went along”) as well as his substance (“He clearly studied hard.”)

Final grade, Romney: A-

Between the first debate and October 10, Romney’s brazen flip-flops were not subject to any serious critique from Time’s political team. What coverage Romney received for altering his campaign positions (aka his “tack toward the political center“) mostly revolved around how conservative activists reacted to Romney’s sudden embrace of moderate rhetoric. (They’re totally fine with it.) Time was much less interested in what the about-faces said about Romney’s candidacy, his character or what his presidency might look like.

The fact that the Republican candidate had radically altered his positions on core domestic issues just one month before Election Day was not treated as a campaign evolution that reflected poorly on Romney. To the contrary, it was largely portrayed as a savvy move by the Republican.

Time’s soft peddling of Romney’s broad reinvention was typical of how the Beltway press has politely covered the candidate’s latest chameleon turn.

Politicians once flip-flopped at their own risk knowing the price they’d likely pay from the hypocrisy-sensitive press corps. Indeed, there was a time when it meant something if a candidate made it clear he didn’t believe what he had been saying on the campaign trial, and the press held that revelation against the candidate. Recall that Al Gore was hammered in the press in 2000 as a politician without any core convictions. And George W. Bush’s 2004 re-election campaign was largely built around calling Sen. John Kerry a flip-flopper; a tag that stuck thanks in part to the press coverage.

But this campaign Romney has mostly skated through his latest reincarnation, as pundits marvel at the political ease and wisdom of his flip-flops: “Crafty” announced The Daily Beast. At BuzzFeed, Blake Zeff suggested Romney had bet his entire campaign on the hope that “the era of the flip-flop as untenable, campaign-ending, non-starter is over.” It if is, Romney has the press to thank.

Just look at how his jarring reversals have been watered down in recent days [emphasis added]:

• “Romney ditched that strategy and repeatedly softened the ideological contrasts with Obama.” [Daily Beast]

• “Romney polished the rough edges” [Los Angeles Times]

• “Behind the new efforts by the Romney campaign to soften his conservative edges” [New York Times]

• “It also meant altering or softening his positions on a handful of bedrock issues.” (BuzzFeed)

Romney abandoning the hard-right persona he crafted and campaigned on for the last four years is ‘softening the edges’? Besides, Romney’s maneuver is no big deal, goes the media narrative, because candidates always shift their beliefs for the general election.

From the Washington Post:

Of course, a second-half pivot is a time-honored maneuver in the political playbook. In a primary campaign, a candidate must play to the passions of the base; as he moves toward the general election, the sensibilities of swing voters become paramount.

Right. But we’re not talking about a “time honored” primary-to-general election pivot happening now. We’re talking about a candidate who was trailing in the polls and who decided in October to reinvent himself. That’s not the norm in American politics, although the press has tried to pretend it is. Note that when Obama did reverse his position on the issue gay marriage in May, he offered a public, detailed explanation as to why. Not Romney. He doesn’t bother to explain his campaign 180s and the press doesn’t seem care.

They’re too busy admiring the “chutzpah,” as Halperin called it.

These tweets last week from Politico reporter Ben White helped capture the media’s admiration for Romney’s flip-flops:

See? Romney’s reinvention, his decision to alter his views on the central issues of the campaign weeks before Election Day, is “brilliant.” The brash flip-flopping doesn’t reflect poorly on the candidate or suggest that Romney’s unsure, or unprincipled, about his positions. Instead, it signals savvy politics and “crafty” campaigning.

For the final push of this campaign Mitt Romney is trying to reinvent himself as a moderate, less-scary Republican. And the press is helping him at every turn.

“Medicaid Is the Real Target”: Mitt Romney’s Priorities, Aid For The Rich, Paid For By The Poor

Since August, when Mitt Romney chose Paul Ryan as his running mate, the two campaigns have fought a fierce battle over who is the most stalwart protector of Medicare. In the first presidential debate, Romney assailed President Obama for his $716 billion in Medicare cuts, and Ryan did the same in last week’s vice presidential face-off. Likewise, the Obama campaign has hit Team Romney for the Ryan plan and its Medicare “premium support”—which, if implemented, would gradually replace traditional Medicare with subsidized, regulated private insurance.

The irony is that—in the short term, at least—Medicare will stay unchanged, regardless of who wins the election. Seniors are among the most mobilized voters in the electorate, and there’s too much political risk involved in making big, immediate changes to Medicare. For that reason, Medicare reform plans on both sides are backloaded and will take time to unfold.

The same isn’t true of Medicaid, the other major federal health-care program. The primary constituency for Medicaid—poor and working-class families—lacks the clout and influence of seniors. And while the Obama administration expanded the program in the Affordable Care Act, it has also made Medicaid a ripe target for conservative cuts to social insurance.

This means that, as Mother Jones’ Kevin Drum pointed out last week, Medicaid, not Medicare, is the actual flashpoint in this election. Romney has promised to “block grant” the program, giving states more flexibility in dealing with eligibility and benefits. Some states would use this as an opportunity to innovate. But as Drum notes, just as many would use it as an excuse to drop health coverage for poor people:

Lots of states, especially poor states in the South, don’t have much interest in experimenting. They just want to slash eligibility for Medicaid. Given the freedom to do it, they’d adopt what Ed Kilgore calls the “Mississippi model,” cutting off coverage for a family of three earning anything over $8,200. For all the talk of fresh thinking and new solutions, what they really want to do is simple: They want to stop providing medical care for poor people.

Admittedly, this is a little speculative. It’s possible—albeit, unlikely—that a future governor of South Carolina or Alabama might want to use the new flexibility to improve services for lower-income people. With that in mind, it’s also worth noting the extent to which Romney’s block-grant plan involves a massive cut to overall Medicaid spending. The Center on Budget and Policy Priorities finds that with a Paul Ryan-style block grant in place, overall Medicaid spending would decline by one-third over the next decade. When you put this in the context of Romney’s budget proposals—which include new defense spending and a promise to protect Medicare—and his promise to repeal the Affordable Care Act, the result is a $1.5 trillion reduction in Medicaid spending by 2022. These cuts would add an additional 14 to 19 million people to the ranks of the uninsured, on top of the 30 million people who would lose coverage as a result of full Obamacare repeal.

It’s his approach to Medicaid, more than anything else, that reveals Mitt Romney’s priorities—aid for the rich, paid for by taking relief from the poor.

“Wrong Again Mitt”: Romney Says People Don’t Die Because They Lack Insurance

In an interview with the Columbus Dispatch in Ohio published Thursday, Mitt Romney repeated a claim that already got him in trouble once this cycle and has reflects an enduring belief among Republicans: that people in the U.S. don’t die because they lack health insurance.
“[Y]ou go to the hospital, you get treated, you get care, and it’s paid for, either by charity, the government or by the hospital,” Romney said. “We don’t have people that become ill, who die in their apartment because they don’t have insurance.”

It’s eerily reminiscent of a statement President George W. Bush made in 2007 that haunted Republicans during the 2008 campaign — “[P]eople have access to health care in America. After all, you just go to an emergency room.”

There’s just one problem: It’s not true.

Numerous studies over the past 10 years conclude that tens of thousands of Americans die each year because they lack insurance.

A 2009 study conducted at Harvard Medical School and Cambridge Health Alliance, and published in the American Journal of Public Health concluded that “[l]ack of health insurance is associated with as many as 44,789 deaths per year in the United States, more than those caused by kidney disease. … The increased risk of death attributable to uninsurance suggests that alternative measures of access to medical care for the uninsured, such as community health centers, do not provide the protection of private health insurance.”

A 2012 report by the health care reform advocacy group Families USA concluded that 26,100 people died prematurely in America in 2010 due to lack of insurance. That report extrapolated from a 2002 Institute of Medicine study — conducted when the uninsurance rate was lower — which concluded that 18,000 people died prematurely because they weren’t covered.

In a 2009 update, the IOM concluded that uninsured patients are at higher risk of mortality or poor health outcomes in the aftermath of both acute medical issues (heart attacks, serious injury, stroke) and chronic ones (cancer, diabetes).

In 2008, the Urban Institute’s Stan Dorn concluded that “[b]ased on the IOM’s methodology and subsequent Census Bureau estimates of insurance coverage, 137,000 people died from 2000 through 2006 because they lacked health insurance, including 22,000 people in 2006. Much subsequent research has continued to confirm the link between insurance and mortality risk described by IOM. In fact, subsequent studies and analysis suggest that, if anything, the IOM methodology may underestimate the number of deaths that result from a lack of insurance coverage.”

Conservatives have attacked these findings and methods and argued that, controlling for health status, there’s no difference in survival probabilities between insured and uninsured people. When the Families USA report came out, Avik Roy, a Romney health adviser, called its findings “statistical hogwash.”

To buttress his argument, he cited a thorough study by Richard Kronick — a University of Rochester health policy expert who served in the Obama administration and was a senior adviser to Bill Clinton during his push for health care reform. His conclusion? “[I]f two people are otherwise similar at baseline … but one is insured and the other uninsured, their likelihood of survival over a 2-16-year follow-up period is nearly identical.”

Further, I show that survival probabilities for the insured and uninsured are similar even among disadvantaged subsets of the population; that there are no differences for long-term uninsured compared with short-term uninsured; that the results are no different when the length of the follow-up period is shortened; and that there are no differences when causes of death are restricted to those causes thought to be amenable to the quality of health care.

However, Kronick conceded that “[g]iven the inherent uncertainties in inferring causality from the results of observational analyses, the results presented here are not able to provide a definitive answer to the question, ‘How many fewer deaths would there be in the United States if all residents were continuously covered by health insurance?’”

In an interview, Urban’s Stan Dorn praised Kronick but defended his and his colleagues’ conclusion.

“I’m aware of Rick’s study and he’s a great researcher. And I guess what I’d say is it’s an outlier,” Dorn said in an interview. “There’s a lot of research that goes beyond what we did, and it’s an outlier.”

Dorn noted that other studies focusing on particular ailments make the link between uninsurance and death quite clear. “We know that women with cervical cancer who are uninsured get their cancer detected later…. We know that people with heart disease don’t take their medicine because they can’t afford it…and sometimes die.”

And as Boston University health economist Austin Frakt noted when he engaged this same controversy in February 2010, “among recent studies in this area the evidence is greater than three-to-one in favor of an insurance-health outcome link, including mortality.”

In 2006, then-Massachusetts governor Romney himself agreed — at least to an extent. Though he did not address the mortality issue specifically, in an April 2006 presentation before the Chamber of Commerce he conceded that uninsured people who seek health care at emergency rooms experience worse outcomes.

“There ought to be enough money to help people get insurance because an insured individual has a better chance of having an excellent medical experience than the one who has not. An insured individual is more likely to go to a primary care physician or a clinic to get evaluated for their conditions and to get early treatment, to get pharmaceutical treatment, as opposed to showing up in the emergency room where the treatment is more expensive and less effective than if they got preventive and primary care.”
