“Just Die Already!”: GOP Would Bar Poor From Health Care Altogether

During a Republican primary debate in the last presidential election cycle, there was a dispiriting moment in which Tea Party audience members cheered at the idea that a comatose uninsured American — unable to afford health insurance — would be left to die. That infamous outburst, among others, has prompted GOP bigwigs to try to cut back on primary season debates, hoping to limit appearances that might expose the party’s baser impulses.

But that mean-spirited and contemptuous attitude toward the sick is alive and well in the Grand Old Party, as its maniacal (and futile) resistance to Obamacare has made clear. Now, one Republican politician is pushing that callousness to new lows: He wants to bar the uninsured from hospital emergency rooms.

Last week, Georgia Gov. Nathan Deal criticized a decades-old federal law that requires all hospitals that receive Medicare funds and have emergency facilities — and that’s most — to treat any patient who walks in needing care, regardless of his ability to pay. “It came as a result of bad facts,” Deal said, according to The Atlanta Journal-Constitution. “And we have a saying that bad facts make bad law.”

Deal says that many people use emergency rooms unnecessarily, and those patients inflate health care costs. He is factually correct. But there are other facts that undercut his arguments and reveal his hypocrisy.

First off, Deal is among those red-state Republicans who have vociferously opposed the Affordable Care Act, which makes health insurance available to hundreds of thousands of people who couldn’t otherwise afford it. If more people had health insurance policies that paid for doctors’ visits, fewer would use emergency rooms for routine complaints.

Second, Deal, like many Republican governors, has refused the Medicaid expansion made possible by Obamacare, even though the federal government would pick up 100 percent of the cost for the first three years and 90 percent until the year 2022. That expansion is the best chance many Georgians without means have for getting health insurance.

So, to sum up, Deal hates Obamacare and refuses its Medicaid expansion, which would keep the working poor out of emergency rooms. In addition, he wants to deny them access to emergency rooms unless they can pay. Really, governor? Don’t you insist that your values are “pro-life”?

It’s no wonder that GOP strategists shuddered when audience members responded so cruelly during the CNN/Tea Party Express debate in September 2011. It portrays the party as pitiless — a reputation unlikely to attract a majority of voters.

Quiet as it’s kept, most Americans support keeping Obamacare, despite the relentless pounding it has taken from Republicans. (And despite a website rollout that was infuriatingly incompetent.) A new poll by the Kaiser Family Foundation found that 56 percent of Americans favor keeping it in place, while just 31 percent want to repeal it. (Twelve percent want to replace it with a GOP plan.)

That’s likely because most voters, no matter their reservations about Obamacare, know that the Republican Party has no good solution for the millions of Americans who work every day but still don’t earn enough money to buy a health care plan. Americans have struggled with the nation’s dysfunctional health care “system,” and they know it’s overdue for an overhaul.

Meanwhile, as the midterm elections draw closer, the GOP struggles to come up with a plan that pretends to overhaul the health care system. Looking to avoid being painted as mere obstructions, House Republican honchos are working to draw their caucus together behind a bill that would replace Obamacare with a workable alternative.

But the most sincere plan so far — one offered by Sens. Richard Burr (R-NC), Tom Coburn (R-OK) and Orrin Hatch (R-UT) — would probably offer policies too skimpy to do any good once a policyholder gets sick.

Besides, even that replacement idea seems unlikely to draw broad support among the far-right Tea Partiers, who believe that allowing the uninsured poor to die is the appropriate government response to the health care crisis.

That’s a hulking bit of hypocrisy for a party that advertises itself as “pro-life.” Deal’s latest proposal is one more reminder of how little that label means.

“Scott Walker’s Little-Known Scandal”: When He Treated Welfare Recipients Like Dogs

Among the racist jokes and emails found in recently released documents connected to the criminal probe of Gov. Scott Walker’s 2010 campaign, one stood out: A “joke” about a woman trying to sign up her dogs for welfare, because “my Dogs are mixed in color, unemployed, lazy, can’t speak English and have no frigging clue who their Daddys are. They expect me to feed them, provide them with housing and medical care, and feel guilty.” The punch line: “My Dogs get their first checks Friday.”

Walker’s deputy chief of staff Kelly Rindfleisch replied: “That is hilarious. And so true.”

The joke is bad enough on its own, but it’s also worth noting: Back when Walker was Milwaukee county executive, and Rindfleisch was a top aide, he managed the county’s welfare programs so abysmally that after lawsuits by local clients, the state was forced to take them over. “They didn’t just call people dogs, they treated them like dogs,” one Milwaukee elected official recalled angrily.

“Milwaukee County has demonstrated a sustained inability to successfully provide services to its (poor) customers,” state health services director Karen Timberlake wrote in a February 2009 letter to Walker announcing the state takeover. Milwaukee became only one of 72 Wisconsin counties to wind up with its programs for poor people under state control.

It’s a chapter in Walker’s career that shows why, to many in Milwaukee, his staff’s racist jokes aren’t funny.

At the height of the recession, in 2008 and 2009, requests for aid in Wisconsin, and throughout the country, soared. But in Milwaukee, where 41 percent of African-Americans live below the poverty line, people had trouble getting help. Roughly 95 percent of calls to the county’s client-intake call center went unanswered in 2008, a state probe later found.

The social services department budget funded 25 positions at the intake center, but a Milwaukee Journal-Sentinel reporter found only seven staffers working among empty cubicles when he visited. Advocates and the county workers’ union complained, but Walker stonewalled. Aided by the outcry, Walker began arguing for privatizing the social services intake unit. “He was managing it to fail,” charges AFSCME contract administrator Dave Eisner.

In June 2008, Legal Action of Wisconsin sued on behalf of thousands of needy people who couldn’t get benefits even though they qualified, because they couldn’t get their eligibility verified.

“Milwaukee County has reached a low point in its [welfare] delivery service,” Legal Action lawyer Pat DeLessio wrote in a letter to the County Board. “It is almost impossible to get through to anyone on the phone” to apply for or verify benefits.

But the problems weren’t just at the call center. In 2008, one out of five food stamp recipients dropped for ineligibility were in fact eligible, and wrongly cut from the program. In 2007, 60 percent of county decisions to cut food or other aid were overturned on appeal within two months. Roughly 30 percent of needy applicants were waiting more than two weeks for aid. Two-thirds of all complaints received by state welfare agencies involved Milwaukee County residents having problems obtaining Medicaid, food aid and child care services. And while the state paid a higher share of Milwaukee’s income-maintenance program costs than in other counties, Walker complained that state funding was inadequate.

With the call center problems and need rising, clients took to lining up at county offices for services before they even opened, DeLessio recalled, because by midday workers would declare the building was full, and turn away new applicants. In June 2008 at least 3,000 people showed up before dawn seeking food vouchers in what was later called a “food riot.”

“The food crisis in Milwaukee and throughout the United States is worse than many of us have realized,” said Milwaukee Common Council president Willie Hines. “We expect long lines for free food in third-world countries.”

Walker’s answer was to privatize the intake unit and other services. His proposed September 2008 budget featured his privatization proposals, but the county board blocked him. “It was clearly a game – he didn’t give a damn about poor people,” Eisner charges.

State officials repeatedly complained about the service inadequacies, and eventually threatened to take the programs away from the county. Politics clearly played a role in the conflict; in 2006 Walker had planned to run for governor against incumbent Democrat Jim Doyle, but soon dropped out of the Republican primary. “I believe that it was God’s will for me to run,” Walker said at the time. “After a great deal of prayer during the last week, it is clear that it is God’s will for me to step out of the race.” It was an open secret that Walker was planning another run for governor in 2010, and if his skirmishes with the Doyle administration hurt Milwaukee’s poor, they helped its county executive with the state’s conservative GOP primary voters.

After a series of tense meetings between county and state administrators, when it was clear the state was going to take over the anti-poverty programs, Walker made a brazen move. He wrote to state social services director Karen Timberlake and invited the state to take over the county’s income maintenance program.

“This is a state mandate,” Walker wrote, in a letter he immediately released to the media. “It’s amazing state government has been such a lousy partner on this.”

County board chair Lee Holloway told the Milwaukee Journal-Sentinel that Walker invited the state in, over his and the board-majority’s objection, to preempt the state’s embarrassing announcement that it was taking over Milwaukee’s programs. “Holloway said he thought Walker’s letter was meant to upstage state officials ‘before they make a move on him,’” the paper reported.

“The county board didn’t want the takeover,” recalls Legal Action’s Pat DeLessio. “There’s a strong system of county control in Wisconsin. But Walker just gave up.”

No one was fooled by Walker’s letter. A day after he released it, the state announced its takeover. In her letter to Walker explaining the move, Timberlake wrote that Wisconsin state government “has in fact expended millions of additional dollars and thousands of hours of staff resources to assist your county over a period of years. Despite these efforts, Milwaukee County’s performance fails national and state standards and is failing the people of the county.” Yet two years after the state took over his social service programs, Walker took over the state as governor.

Kelly Rindfleisch, who found the joke about welfare-receiving dogs “hilarious” and “so true,” was Walker’s deputy chief of staff while he was mismanaging the county’s welfare programs. Her boss, chief of staff Tom Nardelli, himself circulated a racist joke about the “nightmare” of waking up black, gay, disabled and HIV-positive while working for Milwaukee County. Against the backdrop of the way Walker treated welfare recipients, their joking is even less funny.

Rindfleisch was eventually convicted of illegal campaign activity on public time, a felony conviction that she is appealing. She and Nardelli paid no penalty for enjoying racist jokes on public time.

“Obamacare Won’t Cause Society To Collapse”: Americans Choosing To Work Less Doesn’t Mean They’re Losing Their Jobs

A small war has erupted over the recent Congressional Budget Office report on the employment effects of the Affordable Care Act. Last week, the CBO itself felt compelled to offer a lengthy and detailed rebuttal to the spin that millions of Americans will “lose their jobs” as a result of Obamacare.

So let’s first be clear about what the CBO report concluded: As a result of the ACA, millions of Americans will choose to work less, if at all. That doesn’t mean that they will “lose their jobs.” Rather, it means that many will choose to give up working double-shifts just so they can make enough to afford health insurance or leave jobs they hate but have kept simply because they can’t maintain their coverage otherwise. In virtually all cases, these are decisions people are making for themselves and presumably welcome. As the CBO points out, as opposed to “losing their jobs,” in which case we’d all feel sad for them, friends and neighbors will invariably feel happy for these individuals.

But, of course, not everyone. To conservatives, the CBO report demonstrates what they have said about Obamacare – and about the government dole generally – all along: It creates “perverse incentives” encouraging people not to work.

The conservative argument is based on several underlying assumptions, like a DirecTV ad: When you give things to people, they work less. When they work less, they’re worse off. When they’re worse off, they demand more. When they demand more, liberals give them more. And when liberals give them more, society collapses. Don’t have society collapse: Stop the Affordable Care Act.

Of course, the CBO report did in fact find that providing this health coverage will induce millions of people to work less or not at all. So let’s look a little more closely at this syllogism.

It’s undoubtedly true that if you give things to some people, they’ll work less. But it’s not true in all cases. Unfortunately, this sort of assertion is a staple of anti-government rhetoric: For any government expenditure, it can be shown to have enriched some deadbeat or rip-off artist. But so has the derivatives market. Meanwhile, plenty of people work more when you give them more.

In fact, most conservative policies these days are based on the idea that certain people need to be given more to induce them to work harder and to produce more. Of course, those highly-sensitive individuals are the rich and corporate executives, who, without more money (including from the government) simply wouldn’t keep working and creating. By the same logic, though, we should extend even more benefits to more working people – perhaps even raise wages at the low end – to encourage them to work. But for some reason low-income Americans, unlike the wealthy, are presumed to work best if we take incentives and benefits away from them.

Moreover, as the CBO pointed out, there is indeed a “perverse” work disincentive in Obamacare – but it’s the opposite of what conservatives have taken the report to say. Rather, it’s that, as people’s incomes rise, they get less support – a “tax,” in effect, on work. And, of course, taxes are bad. So we actually should be less stingy about giving even better health care benefits to even more people.

Of course, we’d need to pay for those expanded benefits, which appears to be the real point of the “collapse of our culture” argument – not so much that people won’t work as that people who do work will wind up having to support them. But there’s then an obvious way to pay for these benefits if you want to encourage work: tax unearned income (which accrues, by definition, to nonworkers) at a higher rate than we tax earned income. And if giving people money or benefits for which they didn’t have to work encourages sloth, then we’d best start taxing away all inheritance post-haste, as well.

We don’t, of course, because that would tax primarily the rich. But most parents want to leave something behind for their children, because we know that getting a leg up is usually the way to climb even higher. Few people throw their kids out of the house with no means of support, on the grounds that that will make them more successful. Nevertheless, many argue that helping other people’s children only cripples them as opposed to, well, helping them.

It is incumbent upon liberals to assert not just that children “deserve” health care or that people “shouldn’t have to” work grueling hours and still not make ends meet. Such assertions are, after all, merely subjective. But if investments in human capital actually improve total productivity, then the only argument against is that “the poor you always have with you” actually is a commandment, not a condemnation. And various studies (such as this and this) have shown – not surprisingly – that health care is one of the better bets for boosting productivity and workforce engagement.

And productivity, after all, is really the issue. No one longs for the days when people had to toil every waking moment to scrape out subsistence livings, instead of a modern world where a 40-hour work week can enable one to produce more economic value than the greatest medieval monarchs could even dream of. So do we really think it’s good if more and more Americans feel compelled to work 80 hours a week just to make ends meet? Would it mean our economy, or our morals, were headed downhill if more Americans decided they didn’t need to work two shifts every day but could get by, having all they want, on only one?

In short, it isn’t clear that more work is self-evidently good. Or that society will collapse if people work a little less – let alone if it makes them more productive overall – because they have health care. Just as it didn’t collapse when we moved to a 40-hour workweek and ended child labor. But it’s possible. After all, when I can’t get cable, I do get angry.

“A Particularly Cruel Joke”: Texas Lawmakers Celebrate “Achievements” In Women’s Health As Thousands Go Without Care

The consequences of Texas’ sweeping new abortion restrictions are now being felt across the state, but the status of reproductive healthcare in Texas had been dire long before conservative lawmakers passed the omnibus measure to shutter reproductive health clinics, restrict safe abortion services and leave thousands of women without access to necessary care.

Texas lawmakers passed a two-year budget in 2011 that cut $73 million from family planning programs; the following year, Rick Perry dissolved the state’s partnership with the federal Women’s Health Program, forfeiting millions in Medicaid funding for low-income women’s healthcare. Lawmakers restored some of this funding in 2013, but reproductive health providers like Planned Parenthood are barred from receiving it. That Perry has refused the Medicaid expansion has further compounded the crisis that has been building in the state, the blunt impact of which disproportionately impacts low-income women of color.

Republican “reforms” to the system have resulted in a 77 percent drop in the number of women being served by state health clinics at an additional cost of around 20 percent. The maternal mortality rate — particularly among women of color — is on the rise, and Texas has the highest uninsured rate in the nation.

It is in this context that the Texas Health and Human Services committee’s decision to hold a hearing on the “progress” the state has made in women’s healthcare seems like a particularly cruel joke. The committee intends to “build on previous legislative achievements in women’s healthcare,” according to a statement on the hearing.

Activists in the state, who have remained focused on challenging the rollback of reproductive rights in the months since Wendy Davis’ marathon filibuster, descended on Austin Thursday to provide testimony and protest the show hearing.

“When I heard about the hearing — well, I felt like if the Daily Show was going to create a parody, they couldn’t have done a better job,” Amy Kamp, one of the women providing testimony at the hearing, told ThinkProgress. “If Texas wants to protect women’s health, I have a helpful suggestion. Just reinstate the old program we used to have!”

“It’s laughable that the same politicians that have devastated Texas women’s access to healthcare — cancer screenings, birth control, and safe, legal abortion — are now touting their so-called achievements in women’s health,” said Cecile Richards, president of Planned Parenthood Action Fund. ”If that’s what they call help for Texas women, we’ve had quite enough of it.”

“Political Gridlock’s Millions Of Victims”: It’s One Thing To Seek An Advantage At The Polls, Another To Make Innocent People Suffer

In an election year, there are always winners and losers. Rarely, however, are there so many victims.

Legislative gridlock, which was already bad enough, has devolved into a cynical, poisoned status. With a few obvious votes, Congress could improve the lives of millions of people — the unemployed, the undocumented, the uninsured. But instead of being helped, those in need are punished for nakedly political reasons.

It says a lot about this shameful state of affairs that Senate Minority Leader Mitch McConnell (R-Ky.) , one of the most powerful and savvy officials in Washington, had to put his career on the line to win an increase in the federal debt ceiling. Failure to act would have caused a catastrophic default. No new government spending was involved; rather, the Treasury simply needed to pay for spending that Congress already had authorized. Raising the limit was a no-brainer.

Yet Sen. Ted Cruz (R-Tex.), who continues to redefine the word “shameless,” almost led the nation into calamity by forcing Republican senators to go on record in favor of the increase. Since the GOP base has been told — wrongly — that refusing to hike the ceiling would somehow help get the debt under control, senators who voted to do the right thing risked a tea party challenge.

McConnell, who already faces a tough primary contest, sucked it up and did his duty. Cruz grinned and smirked during the vote, then presumably made preparations to receive a flood of tea party campaign cash for his anticipated presidential run.

At least Congress managed to avoid inflicting grievous harm on the entire nation. A number of subgroups have not been so fortunate.

The Americans most obviously suffering because of Congress’s unwillingness to do the right thing are the 1.7 million jobless workers who have lost their long-term unemployment benefits.

Democrats keep proposing legislation to extend those benefits, as has regularly been done in tough economic times. Republicans say they agree but insist — contrary to common practice — that the extension be paid for with cuts elsewhere in the budget.

Again, Republicans are wary of angering the party’s conservative base. It’s not so much a matter of increasing the deficit — a three-month extension would cost only $6 billion, and Democrats have proposed offsets — but that far-right dogma considers such payments a moral hazard that encourages idleness. Never mind that recipients of unemployment benefits, by definition, were employed until relatively recently and can demonstrate that they are actively looking for jobs.

The working poor are suffering unnecessarily as well. The federal minimum wage of $7.25 is far too low. In the past, Republicans have joined Democrats in voting for needed increases. In an election year, however, struggling wage-earners are out of luck.

The 11 million men, women and children who are in this country without documents are also victims of the calendar. President Obama and the entire congressional leadership agree that there is an urgent need for immigration reform.

The Senate has already passed a comprehensive bill that increases border security and offers the undocumented a path toward citizenship. Many observers believe there are enough votes in the House to pass the Senate bill and send it to Obama for his signature. But because of the looming election, that proposition isn’t being tested.

House Speaker John Boehner (R-Ohio) would face a revolt in the conservative GOP caucus if he allowed Democrats and a few moderate Republicans to pass a comprehensive immigration bill. Boehner has established the precedent that he can use this maneuver to avert certain disaster — it’s how he got a “clean” debt-ceiling increase through the House. But his members will not abide being painted as “soft on immigration” in an election year.

Also unfairly punished are the millions of uninsured Americans seeking coverage through the Affordable Care Act. Despite the Republican Party’s best efforts, Obamacare is working. But it would work better if Congress would cooperate with Obama in making a host of technical adjustments to the program.

This sort of after-the-fact tinkering has been required for every big social program. But Republicans have so demonized Obamacare that collaborating in an effort to make it function more effectively would be, for the far-right base, tantamount to treason.

It’s one thing to seek an advantage at the polls. It’s another thing to make innocent people suffer for your ambition. Guilty members of Congress — and I’m specifically including you, Sen. Cruz — should hang their heads in disgrace.
