“Like A Drunk In A Bar Fight”: Why Republicans Will Never Stop Lying About Obamacare
Politically speaking, here’s the thing about those melodramatic ads attacking the Affordable Care Act currently running on TV: In terms of actual policy, they’re as futile as the 40-odd votes to repeal the law that House Republicans have already cast.
GOP hardliners are like a drunk in a bar fight threatening to whip somebody twice his size if only his friends would let go of his arms.
It’s all over but the shouting.
Even if Republicans make big gains in the 2014 congressional elections, they can’t possibly win enough votes to overcome a presidential veto. What’s more, chances of capturing the White House in 2016 on a platform of canceling millions of Americans’ health insurance benefits appear so remote as to be downright delusional. Like it or not, the ACA is here to stay.
Indeed, governors and legislatures in previously recalcitrant states including New Hampshire, Tennessee, Pennsylvania, Utah and Virginia are considering Medicaid expansion they’d previously shunned. Despite early signup problems with the federal HealthCare.gov exchange, signups for individual private policies have increased to where it now appears the ACA will come close to meeting its projected goal of 7 million enrollees by the March 31 deadline.
Moreover, for all the predictions of actuarial doom heard on Fox News and elsewhere—supposedly caused by an imbalance of old, sick enrollees versus younger, healthier ones—the Washington Post reported last month that “the Kaiser Family Foundation estimates that if the market’s age distribution freezes at its current level—an extremely unlikely scenario—‘overall costs in individual market plans would be about 2.4 percent higher than premium revenues.’”
That’s a minor problem, but nothing like a “death spiral.”
In terms of affecting health care policy, then, the TV ads are largely symbolic — scripted melodramas calculated to arouse the partisan passions of the GOP “base” in states where control of the U.S. Senate could be determined this fall. Financed by Americans for Prosperity, the Scrooge McDuck-style front group controlled by the Koch brothers and fellow anti-government tycoons, they’re aimed less at killing the Affordable Care Act than convincing voters that Democrats are their enemies.
Maybe that’s why the ad campaign has proven so singularly unpersuasive to skeptics. In Lousiana, where Democratic Sen. Mary Landrieu is up for re-election this fall, AFP has run a commercial featuring a group of actors pretending to be ordinary Louisiana citizens whose health insurance was canceled due to “Obamacare.” But it’s make-believe; a scripted TV drama as fictive as a Viagra advertisment.
In Arkansas, virtually every news program features a pretty, AFP-sponsored actress plaintively begging viewers to remind Democratic Sen. Mark Pryor that health care is about “people,” and that “the law just doesn’t work.” More in sorrow than anger, it seems, because Pryor remains personally popular.
Pryor’s opponent, Koch-financed Rep. Tom Cotton, tells a touching tale about one “Elizabeth, from Pulaski County” whose premiums have allegedly risen 85 percent under the new law “simply because Washington politicians and bureaucrats think they know what’s best for her and her family.”
I found myself wondering what kind of insurance plan the otherwise unidentified Elizabeth used to have, or if she’s like one of those imaginary digitally enhanced hotties that Internet ads assure me are just a mouse-click away.
Supposedly factual AFP ads have proven even less persuasive to skeptical journalists. In Michigan, 49-year-old leukemia patient Julie Boonstra earnestly explained to viewers that her existing health care policy had been canceled due to the Affordable Care Act, implying that she’d also lost her doctor and been broadsided by ruinous costs.
Fact checks by the Washington Post and Detroit News, however, determined that Boonstra hadn’t lost her doctor at all. What’s more, her monthly premiums under the Affordable Care Act cost roughly half what she’d been paying ($571, from $1,100). Her out-of-pocket expenses almost precisely matched those savings — overall, a wash.
A determined opponent of the law, apart from her understandable anxiety about changing insurance carriers while fighting cancer, Boonstra turned out to have suffered no real losses. Not to mention that she now has a policy that can’t be rescinded due to a “previously existing condition.”
And so it goes. Los Angeles Times economics columnist Michael Hiltzik has made a minor specialty out of fact checking these successive tales of woe. It’s left him wondering if there are really any “Obamacare” victims at all.
“What a lot of these stories have in common,” he writes “are, first of all, a subject largely unaware of his or her options under the ACA or unwilling to determine them; and, second, shockingly uninformed and incurious news reporters, including some big names in the business, who don’t bother to look into the facts of the cases they’re offering for public consumption.”
Politically, however, printed facts rarely prevail against televised fictions. Anyway, repealing the Affordable Care Act isn’t the point. It’s inflaming the GOP base and defeating Democrats.

“A Particularly Cruel Joke”: Texas Lawmakers Celebrate “Achievements” In Women’s Health As Thousands Go Without Care
The consequences of Texas’ sweeping new abortion restrictions are now being felt across the state, but the status of reproductive healthcare in Texas had been dire long before conservative lawmakers passed the omnibus measure to shutter reproductive health clinics, restrict safe abortion services and leave thousands of women without access to necessary care.
Texas lawmakers passed a two-year budget in 2011 that cut $73 million from family planning programs; the following year, Rick Perry dissolved the state’s partnership with the federal Women’s Health Program, forfeiting millions in Medicaid funding for low-income women’s healthcare. Lawmakers restored some of this funding in 2013, but reproductive health providers like Planned Parenthood are barred from receiving it. That Perry has refused the Medicaid expansion has further compounded the crisis that has been building in the state, the blunt impact of which disproportionately impacts low-income women of color.
Republican “reforms” to the system have resulted in a 77 percent drop in the number of women being served by state health clinics at an additional cost of around 20 percent. The maternal mortality rate — particularly among women of color — is on the rise, and Texas has the highest uninsured rate in the nation.
It is in this context that the Texas Health and Human Services committee’s decision to hold a hearing on the “progress” the state has made in women’s healthcare seems like a particularly cruel joke. The committee intends to “build on previous legislative achievements in women’s healthcare,” according to a statement on the hearing.
Activists in the state, who have remained focused on challenging the rollback of reproductive rights in the months since Wendy Davis’ marathon filibuster, descended on Austin Thursday to provide testimony and protest the show hearing.
“When I heard about the hearing — well, I felt like if the Daily Show was going to create a parody, they couldn’t have done a better job,” Amy Kamp, one of the women providing testimony at the hearing, told ThinkProgress. “If Texas wants to protect women’s health, I have a helpful suggestion. Just reinstate the old program we used to have!”
“It’s laughable that the same politicians that have devastated Texas women’s access to healthcare — cancer screenings, birth control, and safe, legal abortion — are now touting their so-called achievements in women’s health,” said Cecile Richards, president of Planned Parenthood Action Fund. ”If that’s what they call help for Texas women, we’ve had quite enough of it.”

“The Importance Of Having Health Insurance”: The 2014 Factor No One Is Talking About — Seniors Are Turning On The GOP
Congressional Republicans have passed a budget, raised the debt limit and punted on immigration reform with one goal in mind. They want to make the 2014 midterm elections about Obamacare.
The party seems to be so confident of this strategy that it doesn’t appear to have any “Plan B,” as The Washington Post‘s Greg Sargent continually points out.
While going all-in on the Affordable Care Act makes sense inside the right-wing mindset, where the law is one Fox News interview from disappearing to wherever Mitt Romney was supposed to go, seniors — America’s most reliable voters — may end up leading a backlash against a post-government-shutdown Republican Party that is even less popular now than when George W. Bush left office.
Undoubtably, the poll numbers for the president’s health law remain low months after HealthCare.gov’s bungled rollout — even though it has helped lead the country to the lowest uninsured rate in five years.
But since the 2010 election, after which real, live Americans began gaining health insurance coverage due to the Affordable Care Act, has there been even one election that has been swayed by Obamacare?
Having been the godfather of the law didn’t cost Mitt Romney the 2012 GOP primary. Having signed the bill into law didn’t cost President Obama his re-election. It didn’t stop Democrats from picking up seats in the Senate and the House. Since 2012, Governor Chris Christie (R-NJ) was re-elected after accepting Medicaid expansion and Terry McAuliffe won Virginia’s governorship with a jobs plan centered upon expanding Medicaid.
In Florida, Democrat Alex Sink narrowly leads Republican David Jolly in a special election to replace Rep. Bill Young (R-FL), who passed away late last year. As Jolly attacks Sink on Obamacare, Sink defends the most popular part of the law — the ban on insurers considering pre-existing conditions — and attacks Jolly on Medicare.
Republicans exploited seniors’ fears of Medicare cuts in 2010 — then voted for the same cuts when they took the House. They also went a step further by proposing a plan to radically remake the single-payer system that provides health coverage to every American 65 or older.
Jolly, a lobbyist, has never officially endorsed or voted for the plan created by Rep. Paul Ryan (R-WI) to turn Medicare into a voucher system. However, nearly every sitting Republican member of the House has.
Ryan’s plan and opposition to Obamacare earned him boos when he spoke at the AARP convention as Mitt Romney’s running mate in 2012. And it was certainly part of the reason he was barely visible in the last few weeks of the campaign.
And since the 2012 election, Republicans’ standing with seniors has only deteriorated.
“In 2010, seniors voted for Republicans by a 21-point margin (38 percent to 59 percent),” Democracy Corps’ Erica Siefert noted in her post “Why Seniors Are Turning Against The GOP,” published months before the government shutdown.
In the latest McClatchy-Marist National Poll, the GOP only had a 4-point margin over Democrats.
The same poll found that 58 percent of adults 45-59 and 54 percent of those 60 and older had an unfavorable view of the president. However, 73 percent of adults 45-59 and 74 percent of those 60 and older also reported an unfavorable view of Republicans in Congress.
Democrats recognize that Obamacare may be a liability and are circulating talking points that call attention to the fact that “65 percent of voters agree with the statement ‘we’ve wasted too much time talking about Obamacare and we have other problems to deal with.’” This aligns with polls that show again and again that most people would rather keep and fix the law than repeal it completely.
But it’s quite possible that the GOP’s stand on Medicare could ultimately be more harmful to their prospects than Obamacare is for Democrats.
Any Republican who sticks with repeal can be charged with wanting to raise prescription drug prices for seniors. Along with eliminating the closing of the Medicare drug “donut hole,” repeal also would erase subsidies that are potentially helping millions of older Americans afford care.
“I just cried, I was so relieved,” said 58-year-old Maureen Grey after using her new plan — purchased with the help of Obamacare subsidies — to visit a doctor.
Adults aged 55-64 make up 31 percent of the new enrollees in the health care marketplaces set up by the law. A new Associated Press report notes that workers nearing retirement have been hardest hit by the Great Recession and are in the most desperate need of what the law offers:
Aging boomers are more likely to be in debt as they enter retirement than were previous generations, with many having purchased more expensive homes with smaller down payments, said economist Olivia Mitchell of University of Pennsylvania’s Wharton School. One in five has unpaid medical bills and 17 percent are underwater with their home values. Fourteen percent are uninsured.
As of December, 46 percent of older jobseekers were among the long-term unemployed compared with less than 25 percent before the recession.
And those financial setbacks happened just as their health care needs became more acute. Americans in their mid-50s to mid-60s are more likely to be diagnosed with diabetes than other age groups, younger or older, accounting for 3 in 10 of the adult diabetes diagnoses in the United States each year. And every year after age 50, the rate of cancer diagnosis climbs.
For many of these Americans, the Medicare guarantee isn’t some distant, theoretical promise. It’s a necessity.
And with Obamacare bridging the gap until retirement, Republicans may find that their decision to make the 2014 election about health care will be as ill-advised as shutting down the government to defund it.

“When Women Have Options”: Obamacare, The Greatest Pro-Life Victory Ever
A new report finds that between 2008 and 2011, the abortion rate hit its lowest point since 1973, when Roe v. Wade declared access to the procedure a Constitutional right.
The rate fell 13 percent to 16.9 per 1,000 women in 2011, down from its peak of 29.3 per 1,000 in 1981.
While conservatives want to claim credit for the decline with onerous restrictions on a woman’s right to choose, the availability of contraception and family planning deserve most of the credit, according to the Guttmacher Institute’s Rachel Jones, the lead author of the study.
“The decline in abortions coincided with a steep national drop in overall pregnancy and birth rates,” she said in a statement. “Contraceptive use improved during this period, as more women and couples were using highly effective, long-acting reversible contraceptive methods, such as the IUD. Moreover, the recent recession led many women and couples to want to avoid or delay pregnancy and childbearing.”
Use of contraceptives has become near-universal and more common among married women than single.
Obamacare’s birth control mandate obligates all insurers to cover contraception without a co-pay, as it does all preventive care. Before the law 28 states already had such mandates in place and 85 percent of large firms offered coverage as part of their plans. Since August 1 of last year, that coverage is now standard for women no matter where they live or how they acquire health insurance.
A Washington University study released last year showed that providing birth control at no cost reduced the rate of abortions by 62 to 78 percent compared to the national rate. Meanwhile the right’s strategy of reducing abortions by requiring sonograms has had no effect on a large majority of women, according to a new study in Obstetrics and Gynecology.
Instead of encouraging the pro-life benefits of Obamacare, conservatives continue to try to minimize them. Republicans want to give employers the ability to opt out of the mandate, asserting falsely that some of the birth control options available induce abortion.
By opposing the mandate, the right reveals that its goal has always been to reduce women’s options, not unintended pregnancies. Obamacare will reduce thousands — if not hundreds of thousands — of abortions and could reduce even more if the Republican-led states that rejected Medicaid expansion would agree to provide subsidized care to approximately 4 million Americans.
Instead, so-called “pro-lifers” enable politicians who deny health insurance to those who need it most, threatening the lives of thousands. If only they were as pro-life as Obamacare is.

“Enemies Of The Poor”: Republicans Are Doing All They Can To Hurt The Poor
Suddenly it’s O.K., even mandatory, for politicians with national ambitions to talk about helping the poor. This is easy for Democrats, who can go back to being the party of F.D.R. and L.B.J. It’s much more difficult for Republicans, who are having a hard time shaking their reputation for reverse Robin-Hoodism, for being the party that takes from the poor and gives to the rich.
And the reason that reputation is so hard to shake is that it’s justified. It’s not much of an exaggeration to say that right now Republicans are doing all they can to hurt the poor, and they would have inflicted vast additional harm if they had won the 2012 election. Moreover, G.O.P. harshness toward the less fortunate isn’t just a matter of spite (although that’s part of it); it’s deeply rooted in the party’s ideology, which is why recent speeches by leading Republicans declaring that they do too care about the poor have been almost completely devoid of policy specifics.
Let’s start with the recent Republican track record.
The most important current policy development in America is the rollout of the Affordable Care Act, a k a Obamacare. Most Republican-controlled states are, however, refusing to implement a key part of the act, the expansion of Medicaid, thereby denying health coverage to almost five million low-income Americans. And the amazing thing is that they’re going to great lengths to block aid to the poor even though letting the aid through would cost almost nothing; nearly all the costs of Medicaid expansion would be paid by Washington.
Meanwhile, those Republican-controlled states are slashing unemployment benefits, education financing and more. As I said, it’s not much of an exaggeration to say that the G.O.P. is hurting the poor as much as it can.
What would Republicans have done if they had won the White House in 2012? Much more of the same. Bear in mind that every budget the G.O.P. has offered since it took over the House in 2010 involves savage cuts in Medicaid, food stamps and other antipoverty programs.
Still, can’t Republicans change their approach? The answer, I’m sorry to say, is almost surely no.
First of all, they’re deeply committed to the view that efforts to aid the poor are actually perpetuating poverty, by reducing incentives to work. And to be fair, this view isn’t completely wrong.
True, it’s total nonsense when applied to unemployment insurance. The notion that unemployment is high because we’re “paying people not to work” is a fallacy (no matter how desperate you make the unemployed, their desperation does nothing to create more jobs) wrapped in a falsehood (very few people are choosing to remain unemployed and keep collecting benefit checks).
But our patchwork, uncoordinated system of antipoverty programs does have the effect of penalizing efforts by lower-income households to improve their position: the more they earn, the fewer benefits they can collect. In effect, these households face very high marginal tax rates. A large fraction, in some cases 80 cents or more, of each additional dollar they earn is clawed back by the government.
The question is what we could do to reduce these high effective tax rates. We could simply slash benefits; this would reduce the disincentive to work, but only by intensifying the misery of the poor. And the poor would become less productive as well as more miserable; it’s hard to take advantage of a low marginal tax rate when you’re suffering from poor nutrition and inadequate health care.
Alternatively, we could reduce the rate at which benefits phase out. In fact, one of the unheralded virtues of Obamacare is that it does just that. That is, it doesn’t just improve the lot of the poor; it improves their incentives, because the subsidies families receive for health care fade out gradually with higher income, instead of simply disappearing for anyone too affluent to receive Medicaid. But improving incentives this way means spending more, not less, on the safety net, and taxes on the affluent have to rise to pay for that spending. And it’s hard to imagine any leading Republican being willing to go down that road — or surviving the inevitable primary challenge if he did.
The point is that a party committed to small government and low taxes on the rich is, more or less necessarily, a party committed to hurting, not helping, the poor.
Will this ever change? Well, Republicans weren’t always like this. In fact, all of our major antipoverty programs — Medicaid, food stamps, the earned-income tax credit — used to have bipartisan support. And maybe someday moderation will return to the G.O.P.
For now, however, Republicans are in a deep sense enemies of America’s poor. And that will remain true no matter how hard the likes of Paul Ryan and Marco Rubio try to convince us otherwise.
